The Reconsideration must be received before the 60th day (from the postmark date of the revocation letter) is exceeded.

Submit a CAP, signed by the Authorized Official, to the address listed on the revocation letter.

The CAP must be received before the 30th day (from the postmark date of the revocation letter) is exceeded.

Submit a Reconsideration, signed and dated by an Authorized/Delegated official or legal representative, to CMS address:
