Common Reason Code Errors

• Claim was submitted and according to CMS records, the beneficiary entitlement for Medicare coverage terminated prior to the first date for services provided on the claim.

Common Reason Code Corrections

• Investigate and check the patient's eligibility, if appropriate, correct & resubmit the claim.
