INVALID PROCEDURE CODE.

Common Reason Code Errors

• One or more of procedure codes listed in UB04 FL 74 A-E, electronic equivalent or on page three of Direct Data Entry (DDE) is invalid.

Common Reason Code Corrections

• Verify that procedure codes being billed are valid for date of service. Verify that they are left justified and resubmit claim. Providers with Direct Data Entry (DDE) can PF9 claim once corrections are made.
