  • Medical record documentation does not support enteral nutrition is required to maintain weight and strength commensurate with the beneficiary’s overall health status.
  • Medical record documentation does not support the medical necessity for a special enteral formula.

  • Documentation was not received in response to the Additional Documentation Request (ADR) letter.
  • Claim was submitted with an incorrect modifier.
