Drug or biological acquired with 340B drug pricing program discount, reported for informational purposes for select entities.

The TB modifier was established by CMS and is required for 340B participating entities for separately payable Part B drugs or biologicals acquired under the 340B Drug Pricing Program

• Effective for dates of service on/after January 1, 2024
• May be used effective immediately, in place of the JG modifier
• Required for use by all entities effective January 1, 2025
