- Medical record documentation does not support a monthly evaluation of the patient's wound(s) or documentation in the medical record which justifies why an evaluation could not be done within this timeframe and what other monitoring methods were used to evaluate the patient's need for dressings.
- Medical record documentation indicates the dressings are for use with a tracheostomy covered under the prosthetic benefit. When dressings are covered under other benefits, there is no separate payment using surgical dressing codes.
