Not Otherwise Classified Codes

To accurately identify and price items billed with Not Otherwise Classified (NOC)/Miscellaneous codes, suppliers must include the following information in the NTE 2400 Field of Electronic Claims or Item 19 of paper claims when submitting a claim for a NOC/Miscellaneous code:

  • Description of item or service
  • Product Name
  • Make/Model of Item

Noridian also suggests adding the Manufacturer's Suggested Retail Price (MSRP) in the NTE 2400 Field of electronic claims or Item 19 of paper claims.

The NTE 2400 Field of an electronic claim is limited to 80 characters; therefore suppliers are encouraged to use our list of suggested abbreviations to condense all of the required information into this field.

Separate instructions are available for drugs submitted under NOC codes. Refer to the specific Local Coverage Determination for instructions regarding the submission of a drug under a NOC HCPCS code. In addition, some NOC HCPCS codes have additional policy specific narrative requirements. Suppliers are strongly encouraged to review the policy requirements for billing a NOC code in addition to these general requirements.

When billing for nuts, bolts, screws, or other small parts, these items are included in the allowance for the accessory with which they are being used. If these items are billed on a separate line using an NOC code, they will be denied as being included in the payment/allowance for another service/procedure.
