This MolDX Local Coverage Determination (LCD) has been revised under contract numbers: 01111 (CA), 01211 (AS, GU, HI, NMI), 01311 (NV), and 01911 (CA, HI & Territories).

Summary of Changes:

Under Bibliography revised the broken hyperlink for the 4th reference, deleted duplicate reference #16, and changes were made to citations to reflect AMA citation guidelines. Formatting, punctuation, and typographical errors were corrected throughout the LCD. This revision is effective on 04/16/2026.
