Description of Issue: Reason Code 37256 is assigning to claims inappropriately. The Shared System Maintainer is aware of the issue and is working on a fix. Impacted claims are being suspended and will be released for processing after a fix is implemented.
Noridian Action Required: Noridian is holding impacted claims and will release them once the fix is installed.
05/26/26 - Noridian released suspended claims on May 7. Reason Code 37256 is setting appropriately on claims. FISS is developing a utility to identify and reprocess all claims rejected in error.
06/08/26 - FISS has developed a utility to reprocess claims rejected in error. Estimated date of 07/06/26 to start reprocessing claims.
06/19/26 - FISS initiated the reprocessing of claims on 06/15/26.

06/19/26 - Review adjusted claims in RTP status. The adjusted claims hit various edits that require provider action such as Reason code 31090. Reason code 31090 requires you to review and correct the utilization as needed, then resubmit the claim, as utilization may have changed since the original submission.
