Reason Code Narrative

THE ADMISSION TYPE MUST BE EITHER 1, 2, 3, 4, 5 OR 9 IF THE BILL TYPE IN ITEM 4 IS 11X OR 41X. FOR BILL TYPES 12X AND 22X, ADMISSION TYPE IS REQUIRED FOR CLAIMS WITH RECEIPT DATE OF 7/1/04 OR GREATER. THIS EDIT WILL ASSIGN FOR TOBS 12X/22X WITH CONDITION CODE 'M1' WITH DATES OF SERVICE ON OR AFTER 10/01/05.

Common Reason Code Errors

  • Claim was submitted without an Admission Type "Form Locator" (FL) 19 of a 1, 2, 3, 4, 5 or 9.

Note: Admission Type must be included on all Types of Bills (TOBs) for receipt date on/after April 4, 2011

Common Reason Code Corrections

  • Correct or resubmit the claim with the appropriate Admission Type from the following list:
    • 1 - Emergency
    • 2 - Urgent
    • 3 - Elective
    • 4 - Newborn
    • 5 - Trauma
    • 9 - Information not Available
