Reason Code Narrative

FOR CLAIMS WITH DATES OF SERVICE 10/01/2000 OR AFTER WITH AN ACTIVE RECEIPT DATE OF 04/01/2004 OR AFTER:

THE CLAIM CONTAINS A LINE (S) WITH PROVIDER SUBMITTED NON-COVERED CHARGES AND A 'GL', 'GY' OR 'TS' MODIFIER IS PRESENT.

Common Reason Code Errors

• Provider submitted an outpatient claim for non-covered charges with HCPCS modifier GL, GY or TS.
  • To appropriately assign liability to beneficiary for payment, service line with HCPCS modifier GL, GY or TS is reported as non-covered.

Common Reason Code Corrections

• No provider corrections necessary. This is a valid processing reason code which does not reflect incorrect billing.
