REJECT CODE FOR C7272 IUR ADJUSTMENT. INPATIENT CLAIM WITH INCORRECT PATIENT STATUS DUE TO TRANSFER TO ANOTHER FACILITY.

Common Reason Code Errors

• Beneficiary began a home health episode within three days of discharge from inpatient care.
• Beneficiary readmitted to inpatient care at a different facility later in same day.
• Wrong patient status was chosen for discharge.
