Common Reason Code Errors

• E-diagnosis code was entered in UB04 FL 69 or electronic equivalent
• E-diagnosis code was entered in UB04 FL 67 or electronic equivalent

Common Reason Code Corrections

• Verify E-diagnosis code billed is not the principal diagnosis code.
