Common Reason Code Errors

Non-OPPS claim is submitted with a Type I add-on code without any of its defined primary codes.

Common Reason Code Corrections

  • Submit a redetermination request to process the add-on code separately; or
  • Submit a redetermination request to change denied add-on code to a separately billable code; or
  • Submit a redetermination request to add add-on code to claim and reprocess
  • Contact NCCI if disputing edit
