The codes included for expanded authorization requirements are E0194 (air fluidized bed), K0005 (ultralightweight wheelchair), L0456, L0457, L0486 (thoracic-lumbar-sacral orthoses), L1833 (knee orthosis), L3761 (elbow orthosis), and L3916 (wrist-hand orthosis).

CMS has also expanded the Required Face-to-Face Encounter (F2F) and Written Order Prior to Delivery (WOPD) List, adding 22 HCPCS codes, including select wheelchairs, orthoses, ventilators, oxygen equipment, and air-fluidized beds. Effective October 28, 2026, a face-to-face encounter must occur within six months prior to the written order.

Before providing services or equipment, verify whether the HCPCS code requires Prior Authorization, a Face-to-Face Encounter, or both, to help prevent delays and claim denials. Review the CMS website for the Required List: Required List.
