Probationary Prior Authorization (PPA) Coming October 15, 2026
CMS will implement the Probationary Prior Authorization (PPA) Program on October 15, 2026. The PPA program establishes a one-year period of enhanced oversight for certain newly enrolled DMEPOS suppliers and suppliers that have undergone a change in ownership. During this probationary period, prior authorization will be required for specific HCPCS codes identified by CMS as being at increased risk for fraud, waste, and abuse. Affected suppliers will be required to obtain a prior authorization decision before furnishing applicable items and submitting claims.
