External Infusion Pumps - Final LCD and Response to Comments (RTC) Article Published

On December 11, 2025, the DME MACs published the following final Local Coverage Determination (LCD) and LCD-related Policy Article (PA):

  • External Infusion Pumps LCD (L33794) and LCD-related PA (A52507)

The LCD incorporates finalized revisions, as a result of the LCD reconsideration process and written comments received during the comment period for the proposed LCD. The final LCD expands coverage for blinatumomab to adult and pediatric beneficiaries one month and older with CD19-positive Philadelphia chromosome-negative B-cell precursor acute lymphoblastic leukemia (ALL) in the consolidation phase of multiphase chemotherapy; clarifies the existing blinatumomab criteria to include the age qualifier of "one month and older" to be consistent with the clinical literature and FDA prescribing information; updates coverage criteria for infusion-based therapy for the treatment of motor control symptoms associated with Parkinson's Disease that are inadequately controlled by non-infusion-based therapy; provides a clear pathway to coverage for beneficiaries who are transitioning from one infusion-based Parkinson's Disease therapy to a different infusion-based Parkinson's Disease therapy; and, adds code J7356 to the HCPCS array.

The posting of the final LCD marks the beginning of the 45-day notice period. The final LCD goes into effect for claims with dates of service on or after January 25, 2026.
