Common Reasons for Denial

  • Claim/service lacks information or has submission/billing error(s)
  • This claim is for an oral anticancer drug, but the National Drug Code (NDC) was missing, incomplete, or invalid. Claims for oral anticancer drugs must be submitted with a valid NDC. Refer to the Oral Anticancer Local Coverage Determination (LCD) and Policy Article for additional information. If your claim did include an NDC, make sure that the NDC was effective on the date of service.
