Low Volume Payment Adjustment (LVPA) Request Submissions Now Being Accepted
Noridian is accepting Low Volume Payment Adjustment (LVPA) requests for the upcoming fiscal year from eligible hospitals.
Who Should Apply
Hospitals that meet CMS eligibility criteria for the Low Volume Payment Adjustment (LVPA) may submit a request for the upcoming fiscal year. Eligibility is based on factors such as:
- Low inpatient discharge volume
- Distance to the nearest IPPS hospital
Additional eligibility requirements are outlined in CMS guidance, including CMS Change Request (CR) 14341 and the Continuing Appropriations, Agriculture, Legislative Branch, Military Construction and Veterans Affairs, and Extensions Act, 2026 (Pub. L. 119-37). Federal regulations implementing the low volume hospital adjustment policy are available in 42 CFR 412.101.
Required Documentation
Submit the following documentation:
- Cover letter on hospital letterhead (email accepted with attachment)
- Most recent Medicare cost report
- Worksheet S-3, Part I, Column 15, Line 14 (total discharges)
- Distance verification
- Google Maps printout to nearest IPPS hospital
- Must include current date
- Any additional supporting documentation

Email your request and documentation to:
Include:
- Facility name
- Provider Transaction Access Number (PTAN)
- Fiscal year requested
Providers receive an approval or denial letter once review is complete.
