How to Resolve Reason Code C7272

Reason Code C7272 indicates that the claim has been Returned to Provider due to an (inpatient claim with an incorrect patient status code due to a transfer to another facility). The causes for this error include the following:

  • Beneficiary began a home health episode within three days of discharge from inpatient care.
  • Beneficiary readmitted to inpatient care at a different facility later in same day.
  • Wrong patient status was chosen for discharge.

How can the provider resolve this error?

Check with the beneficiary, the Interactive Voice Response (IVR) System, or the Noridian Medicare Portal (NMP) for the patient’s discharge history. If another facility has a patient status code that is causing the error, their contact information can be found by entering the NPI through the National Plan and Provider Enumeration System (NPPES).
