This MolDX Local Coverage Determination (LCD) has been revised under contract numbers: 02101 (AK), 02201 (ID), 02301 (OR), 02401 (WA), 03101 (AZ), 03201 (MT), 03301 (ND), 03401 (SD), 03501 (UT), and 03601 (WY).

Effective Date: April 16, 2026

Summary of Changes:

Under Bibliography revised the broken hyperlink for the 4th reference, deleted duplicate reference #16, and changes were made to citations to reflect AMA citation guidelines. Formatting, punctuation, and typographical errors were corrected throughout the LCD. This revision is effective on 04/16/2026.
