• Initial applications are out of Jurisdiction Specialty and will not be handled by Noridian.
  • If Noridian does receive an initial enrollment application, the application will be returned
  • Noridian will process change, revalidation and termination applications for provider based HHA's that are currently enrolled with Noridian
• Services must be furnished in a place of residence used as the patient's home
