HCPC PRICING INDICATOR = 'Z' WHICH REQUIRES MANUAL PRICING. THE RATE MUST BE ENTERED ON CLAIMS PAGE 03 BY THE FISCAL INTERMEDIARIES.

Common Reason Code Errors

• A claim is submitted that contains an unlisted HCPC and does not have name of service or test performed in Remarks field.

Common Reason Code Corrections

• Claims for HCPCs in range of 814XX should be submitted with units and charges for each molecular pathology laboratory test performed on a separate line and name of each test provided in remarks field.
• Claims for unlisted HCPCs should contain a description of what was provided for under code in Remarks field.
