Reason Code Narrative

INPATIENT CLAIM WITH INCORRECT PATIENT STATUS DUE TO TRANSFER TO ANOTHER FACILITY.

Common Reason Code Errors

  • Beneficiary began a home health episode within three days of discharge from inpatient care
  • Beneficiary readmitted to inpatient care at a different facility later in same day
  • Wrong patient status was chosen for discharge

Common Reason Code Corrections

  • Verify and correct patient discharge status to reflect appropriate discharge

If you are unable to determine where patient was discharged, check with beneficiary, Interactive Voice Response (IVR) system or call the Provider Contact Center.
