Description of Issue: The Centers for Medicare & Medicaid Services (CMS) identified a system issue leading to inaccurate payments to some physicians and practitioners included on the QPP eligibility file for claims processed January 1, 2026, through February 26, 2026. The system issue has been corrected. The impacted claims have been identified and will be reprocessed.

Noridian Action Required: Claims will be reprocessed. No further action is required from providers at this time.

Provider/Supplier Action Required: Providers should review the adjusted claims on their Remittance Advice (RA), and refund or collect any coinsurance or deductible amounts that may have been altered.

Proposed Resolution/Solution: Claims will be reprocessed via mass adjustments.
