Under CPT/HCPCS Codes Group 1: Codes added 0630U. This revision is due to the 2026 Q2 CPT/HCPCS Code Update and is effective 4/1/2026.

Under Article Text revised 1st bullet to remove "81479-unlisted molecular pathology procedure". Revised 5th and 8th bullets to remove "DEX Z-Code™" and replaced with "DEX Z-Code®". Added "NOTE: When entering the DEX Z-Code on the SV101-7 documentation field for Part B claims please do not add additional characters and/or information on the line" and "Specific tests that have been evaluated for compliance with coverage criteria set in policy (and are thus covered or not covered) can be found in the DEX® registry". Under CPT/HCPCS Codes Group 1: Codes deleted 81479. This revision is effective 4/1/2026
