The Advancing Chronic Care with Effective, Scalable Solutions (ACCESS) model is a 10-year voluntary Innovation Center model that tests whether an alternative payment methodology, called Outcome-Aligned Payments (OAPs) for technology-enabled chronic care, reduces expenditures while enhancing quality of care for Medicare beneficiaries. The model is scheduled for implementation on July 1, 2026; however, claims processing will not start until August 3, 2026.

The ACCESS model will utilize the HCPCS codes G0669-G0678. All ACCESS model HCPCS codes can be billed with telehealth place of service (POS) codes 02 and 10.

Payment for OAP HCPCS G-Codes G0669–G0675 will be nationally priced. Payment for OAP HCPCS G-Codes G0676-G0678 will be GAF priced.
