This MolDX Local Coverage Determination (LCD) has been revised under contract numbers: 01112 (NCA), 01182 (SCA), 01212 (AS, GU, HI, and NMI), and 01312 (NV).

Summary of Changes:

Under Bibliography revised the broken hyperlink for the 4th reference, deleted duplicate reference #16, and changes were made to citations to reflect AMA citation guidelines. Formatting, punctuation, and typographical errors were corrected throughout the LCD. This revision is effective on 04/16/2026.
