Hepatitis B Virus (HBV) Vaccine and Administration
Medicare covers the hepatitis B vaccine and its administration for those individuals who are at high or intermediate risk of contracting hepatitis B. The regulations that list those groups who are determined to be at high or intermediate risk of contracting hepatitis B are found at 42 Code of Federal Regulations (CFR), Chapter IV, Subchapter B, Section 410.63.
CMS revised those billing requirements and a physician’s order is no longer necessary for the administration of a hepatitis B vaccine under Part B. Beneficiaries will no longer be required to obtain a written order from a physician in order to receive the hepatitis B vaccine.
Intermediate Risk Groups include individuals who have not previously received a completed hepatitis B vaccination series and individuals whose previous vaccination history is unknown. An assessment of an individual’s vaccination status can be made with clinical expertise of a physician.*

- Heterosexually active persons with multiple sexual partners (who have had at least two documented episodes of sexually transmitted diseases within the preceding 5 years)
- Staff at institutions for with intellectual disabilities
- Workers who have frequent contact with blood or blood-derived body fluids
High risk
- End Stage Renal Disease (ESRD) patients
- Clients of institutions for with intellectual disabilities
- Someone who lives in same house as a hepatitis b virus carrier
- Homosexual men
- Someone who injects illicit drugs
- Hemophiliacs who receive Factor VIII or 1X concentrates
- Pacific Islanders Medicare beneficiaries who reside on Pacific islands under U.S. jurisdiction, other than residents of Hawaii)
- Persons diagnosed with diabetes mellitus
Medicare beneficiaries currently positive for antibodies for hepatitis B are not eligible for this benefit
