The CMS Medicare Learning Network (MLN) has updated the Evaluation and Management (E/M) Booklet to include HCPCS code G2211 as an add-on code for home visit services. In addition, CMS revised the code description for G0136, changing the focus from social determinants of health risk assessment to a physical activity and nutrition assessment. Services that meet the requirements for G0136 may be performed in conjunction with E/M visits, Annual Wellness Visits (AWVs), or behavioral health office visits.

CMS also published the CMS MLN Advance Care Planning Fact Sheet providing clarification related to E/M codes 99497 and 99498.
