Check Your Application Status First
Before contacting Provider Enrollment, use the Enrollment Application Status Search tool to check the status of your enrollment application. This self-service tool allows you to view application progress using your Application Reference Number or Web Tracking ID. Many status-related questions can be answered online without calling customer service.

Have the following information available to help us assist you more efficiently:
- National Provider Identifier (NPI)
- Provider Transaction Access Number (PTAN), if assigned
- Taxpayer Identification Number (TIN)
How We Can Help
Customer service representatives can assist with up to three enrollment-related questions during each call. Representatives can help with:
- Enrollment application status
- Medicare revalidation inquiries
- PTAN status and effective dates
- General enrollment questions
- Assistance identifying the appropriate enrollment form or resource
- General guidance for completing enrollment applications
Important: Customer service representatives cannot assist with PECOS password resets or account access issues. Contact the PECOS External User Services (EUS) Help Desk for login assistance.

- Corrections or additions to a pending application
- Requested supporting documentation
- Requests for historical enrollment information
- Corrective Action Plan (CAP) requests
Always include your application reference number on correspondence related to a pending application.
