  • Doctor of Podiatry or Podiatric Medicine
  • Doctor of Optometry
  • Doctor of Dental Surgery or Dental Medicine
    • Dental Anesthesiology
    • Dental Public Health
    • Endodontics
    • Oral and Maxillofacial Pathology
    • Oral and Maxillofacial Radiology
    • Oral Medicine
    • Orofacial Pain
    • Orthodontics and Dentofacial Orthopedics
    • Pediatric Dentistry
    • Periodontics
    • Prosthodontics

Non-Physician Practitioners Eligible to Opt Out

  • Physician assistant
  • Nurse practitioner
  • Clinical nurse specialist
  • Certified registered nurse anesthetist
  • Certified nurse midwife
  • Clinical psychologist
  • Clinical social worker
  • Registered dietitian or nutrition professional
  • Mental health counselor
  • Marriage and family therapist

Providers Not Eligible to Opt Out

  • Clinics, groups, and organizations
  • Part A enrolled providers or suppliers
  • Chiropractors
  • Anesthesiologist assistants
  • Speech-language pathologists
  • Physical therapists
  • Qualified audiologists
  • Occupational therapists
  • Any specialty that is not eligible to enroll in Medicare
