- Doctor of Osteopathy (DO)
- Doctor of Podiatry or Podiatric Medicine
- Doctor of Optometry
- Doctor of Dental Surgery or Dental Medicine
- Dental Anesthesiology
- Dental Public Health
- Endodontics
- Oral and Maxillofacial Pathology
- Oral and Maxillofacial Radiology
- Oral Medicine
- Orofacial Pain
- Orthodontics and Dentofacial Orthopedics
- Pediatric Dentistry
- Periodontics
- Prosthodontics
Non-Physician Practitioners Eligible to Opt Out
- Physician assistant
- Nurse practitioner
- Clinical nurse specialist
- Certified registered nurse anesthetist
- Certified nurse midwife
- Clinical psychologist
- Clinical social worker
- Registered dietitian or nutrition professional
- Mental health counselor
- Marriage and family therapist
Providers Not Eligible to Opt Out
- Clinics, groups, and organizations
- Part A enrolled providers or suppliers
- Chiropractors
- Anesthesiologist assistants
- Speech-language pathologists
- Physical therapists
- Qualified audiologists
- Occupational therapists
- Any specialty that is not eligible to enroll in Medicare
