For each NMP inquiry, choose the Tax Identification Number (TIN) or Social Security Number (SSN), National Provider Identifier (NPI) and Provider Transaction Access Number (PTAN) combination under Provider/Supplier Details. The Program will populate depending on the combination chosen. DME users must indicate which Jurisdiction (JA or JD) the inquiry will be performed in. The combination will be auto-populated if there is only one combination on the user account.
